Why are hotels expensive in Europe?

I've seen various travel shows that show hotel rooms in Europe that look like ones in American cities and they quote the price as 400 Euros and its a good price. When I say rooms, I mean the standard bedroom in American hotels and what ever the equivalent is called in Europe. I ask this question because three times the price of a good hotel in Europe is a decent vacation in the United States (Ex Hawaii). I know there are cheaper accommodations and I'm not looking for recommendations, I just want to know why this is the way it is.

A lot of what you see on the travel shows are luxury hotels and/or rates during special festivals like Carnevale in Venice. I lived in Italy for the last 13 years and traveled most weekends and holidays. Places I stayed in Europe were generally comparable to the places I stay when I'm out exploring in the US. the place I stayed in the Netherlands last month to see Keukenhof Garden and the coronation was a little under $100 per night and was very nice.

You must be looking at the very high-end hotels. I have never stayed at anyplace that cost that much per night. I tend to stay in budget hotels, which are family-run and miles away from any horrible sterile hotel chain jobs.
