Just tell them what you want just like you typed it and the size. The only time people look stupid when ordering is when they do not know what they want and they stand in front of the register trying to figure it out while other people are waiting to order. The biggest problem you may have is having to repeat yourself because they may not be able to catch that many additions the first time you tell them.

Just say it like that and as far as small med large goes I don't use the fancy lingo I just tell it like it is small med large. it is what it is.
