This may be silly, but when women with pierced nipples lactate does milk flow out the pierce hole?

    Yes, that is IF they pierced the milkduct.

    There are multiple ducts that carry the breast milk to the surface of the nipple. If one or more of those are pierced (which is very likely), then yes, the milk will leak from the piercing.
