Your question has many variables, almost too many. However without specifics as to disability or age of the pupil, I have to make very general statements.
The special education teacher and any associated specialists required by the student. There can be input by the parents. They are part of the IEP meeting. Regular Education teachers may have input, provided the student is mainstreamed for any part of the day.
