I'm sure they have one but I have never read nor heard about it. If not then they really don't care about them.

Those states all have Republican governors with Republican controlled legislatures, their main reason is political

No state can refuse to offer insurance exchanges but those who refused to set them up had the federal government do it or there was a joint exchange between state and federal.
Many of the Southern Republican states have put obstacles to people getting information like preventing the Navigators from helping people at clinics and hospitals. What a shameful and hateful action to stop people from getting health insurance.
I think you mean states who refused to expand Medicaid. Those states carry on as they are now, with Medicaid funding at the usual 50/50 split and eligibility for people under 21 and over 65 or disabled, on SSI, single parent, new emigrant, blind, etc.
Those who cant afford insurance wont pay a tax penalty, but perhaps when it goes up in 2015 and 2016 they may buy insurance then because the penalty will be much higher. Until then, they will continue as they are going to clinics for care and the ER which is a big reason for Obamacare so the government doesnt have to dish out money for unpaid ER care and hospitals dont raise their rates to cover their losses.

Since obama-no-care is a federal disaster, the federal government should be doing all of the work, NOT pushing it onto the states.

WELL ACTUALLY, WHAT SOME GOP STATE GOVERNORS ARE DOING IS NOT ACCEPTING A / THE MEDICAID EXTENSION PLAN TO BE OF ASSISTANCE TO THE POOREST PEOPLE IN THEIR STATE. AND EVEN THOUGH THE OPEN ENROLLMENT HAS STARTED, IT ALSO SEEMS TO ME THAT THE SAME ARROGANT GOP GOVERNORS ARE HAPPY TO HAVE THEIR BENEFITS BUT DON'T GIVE A DAMN IF MANY OF THE LOWER INCOME PEOPLE WHO ARE PAYING THEIR (GOP) SALARIES DON'T HAVE OR WOULDN'T HAVE QUALITY HEALTH CARE.
