Failure incidences with the Power Split Device (PSD) have been low. The majority of them last the life of the car (~300,000-400,000 miles). Toyota recommends changing the fluid in the PSD at 100,000 miles, although some prefer to do so sooner.
The PSD is not a belt-driven CVT. It is just a planetary gear set, more similar to a differential in an axle than to an automotive transmission. It is very simple and reliable.
I just read that Luscious Garage (a well-known hybrid repair facility) has seen some PSD problems between 150-250k miles, but they receive a large percentage of the small number of problem cases (including taxi fleets that have severe use). With normal driving, failure rates are low. Most of those with over 300,000 miles have not experienced any PSD issues.
