I'm gonna be going to my doctor to get a note?

I'm currently in talks with someone, basically to get financial help with my situation. They said I need to go to my doctor to get a note, basically proving that I have what I say I have.

I'm going to my main doctor (although he isn't the one that originally diagnosed me, he can look up my medical record and just write what's on there).

Any tips on this? I don't even know how to ask him, I just booked an appointment without saying the reason. Should I just go in and say I need a note? Do I need to bring the note with me? What is the standard process?

    You have to make an appt. and talk with a dr. to get a note. You cant get a note without an appt.

    Note should say something like: 'My patient (your name) has suffered with (disease name) for several years and is unable to (whatever you cant do).

    I would appreciate any assistance you could give him, such as (what would help you)

    If you have any questions, please do not hesitate to contact me.

    Doc's name, address, phone and email."
