Hotel Reservation for Schengen visa?

Question

Me and my girl friend shall be staying 2 days in Spain, 4 days in Italy, 3 days in France, now we have book most hotels using my name, where the Visa application in Italy (which we stay at longest) stated below:

"Hotel reservation OR an original letter of invitation from your host in Italy, signed by him/her and with a photocopy of his/her valid I.D. or residence permit in Italy if non-EU-citizen. NOTE – if accommodation booking does not show the name of the person travelling, the person booking the accoomodation or named person on the accommodation documents should provide a confirmation letter stating they have made the booking on the applicants behalf, and also provide copies of their main passport page with photo and signature. The same is also required if the person booking the accommodation is travelling with the applicant."

Now I wonder, what type of letters should I be writing, since I could not find any form online and I am not quite sure how should I write this letter, if anyone did this before please please kindly help us.

PS. We have 10 hotels, which only stated my names, so writing every hotel request to include my gf's name would be pain in the ***, so we chose to write a letter confirmation for each other instead.

Can i unbook a hotel during visa application for italy? I would really love to know because i dont like this hotel behaviour. thank you
