Form number for late tax filing? (Extension past April 15)?

What is the IRS form number for notifying IRS of planned late tax filing, ie,an extension of the April 15 deadline? (Hopefully with the number, I will be able to locate the form.) Thanks.

    It is only an extension to file the paperwork not an extension to pay. Form 4868

    it can be found at www.irs.gov, and should be in the 4000 series
