Is there a site that I can use to collect contact and information?

Is there a site I can use to collect information from partners? I would provide them a link which they could use to provide the following details. Then I could export into a CSV or similar file.

Company Name

Desired Products (this would be check-boxs)

Contact name

Contact email

Contact address

Contact phone

    Perhaps a shared Google docs spreadsheet, where everyone would have edit capabilities.

    Your data looks like a standard marketing opt-in form you could find a java/php script to put it on your own site & database.
