1) Bangladesh: Needs much more experience, they have won very few ODI matches against the top 8 teams. It will take a lot to become World Champion.
India: Needs to improve upon bowling. Although Indian bowlers are doing a much better job, India needs to focus on consistently producing and polishing such bowlers.
Pakistan: Need to improve their batting. The top order may be good but they need to show more consistency. On many occasions, I see top order batsman getting out early and leaving the job to the middle order.
2) Bangladesh: 5 Batsmen, 3 All Rounders (2 spinners & 1 fast/medium bowler) and 3 bowlers (2 fast/medium bowlers and 1 spinner)
India: 4 Batsmen, 3 All Rounders (all spinners) and 4 fast/medium bowlers. More bowlers should be kept as substitutes.
Pakistan: 6 batsmen, 2 all rounders (spinners) and 3 bowlers (all fast/medium). More batsmen should be kept as substitutes.
I've kept more bowlers in India and more batsmen in Pakistan to cover up their weaknesses. Bangladesh is a balanced side but each player needs to perform better. I'm giving more priority to fast bowlers over spinners because they can be more effective on the fast bouncy pitches of Australia and New Zealand.
3) Maybe not if the batsman is in the middle order but if you change the position of an inform opener, that may affect his form. This really differs from player to player.

Problems with Bangladesh is that they suck at an elite level. They should be an associate team.
India have no frontline bowlers
Pakistan is too inconsistant
