I think you want Lexicon Balatronicum, originally compiled as A Dictionary of the Vulgar Tongue by Captain Grose. A Dictionary of Buckish Slang, University Wit, and Pickpocket Eloquence. New edition made in 1811. I've got a copy of it. There was a facsimile edition published by Macmillan in 1981.

As Cara says, this is Captain Grose's Dictionary of the Vulgar Tongue.

As it's long out of copyright, there are various editions scanned and available free online.

Here's the 1785 edition at Google Books http://books.google.co.uk/books?id=zLAJs0_prpwC&pr...

That's it; thanks! I had a copy about fifty years ago and lost it along the way.
