Question about finding a doctor?
Hello,
I am in the process of looking for a family doctor since my paediatrician can no longer see me. A health centeer associated with a larger hospital received good reviews from residents saying they are always fast and professional, so I am considering going there. However, I have a question that is not on their website. They have a contact form, but they want me to provide my address and phone number. Since I have not made a commitment to them and I just want to know something simple, I refuse to give them my phone number and mailing address. I don t mind giving my email, but no phone number. The address and phone number have a star by them, so they are required. Therefore, is it acceptable to put N/A in those fields since I really don t want them having that information? Thank you.

Why don't you just call them? Then you can ask without having to prove contact information.
