Can anyone help me find an MS Excel Form?
I am looking for an Excel form created using macros. A form that has four to five fields and a submit button. Whatever information is entered in those fields and submitted should be collected in another excel sheet.
if anyone knows a place on the internet where i can find such a form ready made, please let me know.

One does not create VBA userforms with macros, if you are speaking of an actual userform and not simply a worksheet.
It is possible to create a VBA userform in a workbook and export it to any specified file location. While I have not exported a form and then sent it via email to be imported, it should be doable. Do you just want the form to add to an existing workbook, or do you want the workbook containing the form?
It would also be necessary to know which worksheet/columns you would wish the data in the form copied to.
