Have you tried changing the dogs diet?
Sometimes hot spots are caused by something in the dogs diet that they become allergic to.
There are foods on the market that are specially formulated for dogs with allergies.
But if these hot spots are seasonal, the dog may be allergic to some plants.
Or even something in your home.
A vet can give you treatment that may help clear up the hot spot.
"One" hot spot is unusual, as they often break out over an area of the dogs body. Are you sure its a hot spot?
You really should have it diagnosed at the vets.
The appropriate treatment can be given. otc treatment is rarely successful.

That's because OTC products don't work, can harm, and even kill your dog.
Take it to a vet for a proper diagnosis/treatment.
