There is no need to travel between stadiums now, but if you did the #4 train runs to Yankee Stadium and the #7 to Citfield. The two cross at Grand Central, Simply take the Manhattan bound train from either venue to Grand Central and then take the outher either Qweens bound or Bronx bound.

The other directions to go to Times Square is just wrong. You would pass the #4 rain connection and add an extra twenty minutes to your tripo.

The #7 train goes to Citifield, and the #4 train goes to Yankee Stadium, you will need to take the 7 train into Manhattan (Times Square) then take train to 59th St and change to the 4 train there.

If you're going from the Bronx to Citifield, you'll be better off driving.
