Can someone have a stroke and not be aware of it?

    No. There always deficits. 1 in 3 will die, 1 in 3 will take a long time to recover and will have permanent damage, 1 in 3 completely recover. So some strokes can be very 'minor' But a blood clot in the brain is not minor.
