Horizon Blue Cross Blue Sheild Insurance Card?

We Have Horizon Direct Access Gold 100/80/60. I've tried to google what the numbers mean but I can't find the answer anywhere. Can anyone tell me what the 100/80/60 part refers to?

    The 100 means they pay 100% of the charges (after you pay the co-pay) for procedures not subject to the deductible.

    The 80 means they pay 80% of the in-network charges for procedures subject to the deductible after you've paid the deductible until you've paid the policy out-of-pocket maximum.

    The 60 means they pay 60% of the out-of-network charges for procedures subject to the deductible after you've paid the deductible until you've paid the policy out-of-pocket maximum.

    You can ask your HR department for more information.
