Yes it is normally all made to look like they are doing the deed but then the only ones who really know are them doing the deed lol

Actors and actresses know what the part requires and so most learn to live with that then there are some where the lead runs off with the other member (s) of the cast (or crew) but this is just the way humans are

They don't actually have sex in those scenes. Unless the scene calls for genitals to be seen, there are skin colored sticky patches that they wear to conceal themselves. This gives the appearance of them being nude when they actually are not.

It's a mixture of camera angles and acting that makes it look as convincing as it does.

Even still, affairs are a very real possibility. Even when a show or movie does not have such graphic sex scenes, working close and portraying a romantic relationship with another actor has created affairs in the past. Meg Ryan and Dennis Quaid actually got divorced over something like that. She did a movie with Russell Crowe where the two of them had chemistry between their characters. It resulted in a real affair, ending the 10 year marriage.

Sometimes they're both naked on set. Sometimes women wear modesty patches and men wear modesty pouches to cover their privates, but it's hidden from the camera. As for actual intercourse, that's so easy to fake and act out that I'm surprised this had to be asked.

A few actually show sex acts........that are clearly NOT faked........they film the real deal as part of the movie.

needless to say, these are generally NOT mainstream movies.......and usually arthouse , arty farty flicks.
