An example of institutional racism is the college education system that places a value system based on your race and gender.
Basically if you are a non-white and non-Asian female, you will be at the top of the list. That is indeed racist and sexist because it places your race and gender as a factor in determining your worth to the college.
That is why the university system should not be asking for race or gender, assuming it's a two-gender college.
