What can I do about this?

Ok so this home improvement company agent came to my house and talked to us about his company about how great they were and everything being high quality and stuff so I decided to go with them and get some insulation done on my roof ok but here's the thing he told me that I needed to sign an authorization form so they can get the other half of the loan but the crazy thing was that when I called them to see where I needed to sent that form to they said that the loan was already authorized... without my authorization? So I threw it away... And then the agent was going to give me a $50 olive garden gift card and well i never got that... what can I do? Should I report this or what?
