Are these legitimate request for claiming prizes?

My fiancé just received an e-mail from GSN (Game Show Network) claiming the she had won a prize from their new game show Winsanity. It was something that she did actually sign up for, and the show does give away prizes. They game a phone number which is affiliated with GSN, but they are asking for a W-9 tax form and a signed affidavit to be submitted to them within 72 hours or she will forfeit her prize. Obviously these forms contain very sensitive information and I'm trying to make sure she isn't putting herself at risk for identity theft or any other same. Does anyone, preferably with legal knowledge, know if this sounds legit?

    Here is their website with their contact numbers. Have your fiance call them ASAP and tell them about the letter and ask if it's legit or not http://www.corp.gsn.com/contact

    They will be able to put her in touch with the right people who can confirm if this is legit or a scammer using their name

    You CAN'T win a contest you don't enter.

    The e-mail is a SCAM. The ONLY prize is your Identity. You DON'T want it claimed.

    Did she just sign up or actually participate in anything to win? A Google search of the game says "Winsanity is the only game show where the audience is actually in the game." So I don't think just putting in an email would get her a prize. Hopefully someone else can help you out.
