Nuclear war is definitely one of the roads that the world can take; however, the chance is very slim. First is the pragmatic aspect; only several countries have nuclear weapons: the United States, Russia, the United Kingdom, France, China, India, Pakistan, Israel, and North Korea. Out of these, North Korea is the only one producing nuclear missiles (and at a very slow pace); all others are either keeping their stockpiles or dismantling them. However, there are enough nuclear weapons to destroy the world several times over, not to mention the heavy nuclear fallout that would result from even several nuclear missiles (especially Russia's since they designed their rockets to have bigger impact radius...but I digress), which would mean that the offensive country would be committing suicide. Therefore, this deterrent factor definitely discourages any route to nuclear war.
Another reason behind the small chance of nuclear war is because of political factors. North Korea is a prime example of this. After several successful attempts at detonating small nuclear explosives, North Korea has heavy sanctions placed against it by the US and other countries, and even its staunchest ally, China, has distanced themselves from the dictatorship, which has led to the starvation and poverty of Kim Jong Un's populace. Therefore, it is fair to say that the country that threatens nuclear war will probably be alienated - even the US with its thick blanket of alliances and peace treaties - simply because other countries would oppose such use of weapons.
Finally, technology today has reached a point where nuclear missiles can be detected and shot down safely. Unlike the 1950s and 1960s, where 24 hour nuclear bombing crews were on constant alert in Russia and United States to retaliate rather than protect, there are more sophisticated means of deterring nuclear war, such as antimissile fighter aircraft, as well as advanced antimissile defense systems such as the Terminal High Altitude Area Defense the United States is deploying against North Korea. As a result, if one country gives the order, the main powers such as China, the US, and Russia are more than able to disable the missiles, as well as fire some of their own.
There are plenty of other reasons, such as the cost of maintaining a nuclear arsenal, as well as nuclear waste, that definitely makes nuclear war almost impossible. However, I hope that my rambling gives plausible reasons why nuclear war will probably not happen (luckily!).
PS If you are really bloodthirsty, nuclear war may happen if a small contingent of people are isolated on another planetary body, such as the moon, while the earth nukes themselves to bits, since any nuclear material is prohibited in space according to an international agreement. However, the same reasons I explained above will probably have the same effect on planetary colonies. Oh well. I think I wrote enough for 15 minutes.

It has already occurred, but on a small scale, in 1945.
Nobody can tell if nuclear war will occur again, but if it does it might be catastrophic for the human race.

Yes. It will occur. What happened in Hiroshima and Nagasaki, as bad as those bombings were, will pale in comparison to the nuclear horror that's coming.
