Function with roots 1 and 2?

Looking for a function with roots 1 and 2 , slant asymptote x+3 and a vertical asymptote at x=5

    f(x) = x+3 + (ax+b)/(x-5)^2

    Let f(x) = x+3 + (kx+h)/(x-5)^2

    Since f(1) = f(2) = 0

    then 1+3 + (k+h)/(-4)^2 = 0

    and 2+3 + (2k+h)/(-3)^2 = 0

    We have 4 + (k+h)/16 = 0

    5 +(2k+h)/9 = 0

    64 + k+h = 0

    45 + 2k +h = 0

    19 - k = 0

    k=19

    Since 64+19+h = 0

    h = -83

    Function is (x+3) + (19x-83)/(x-5)^2
