Sometimes if they are unable to fit into a single seat, some airlines require them to pay for a second seat.
They shouldn't have to.. they really shouldn't have to.

There HAVE been cases where grossly overweight passengers were required to buy two tickets because they couldn't fit their bulk into just one seat.

Have to pay twice as much? Depends on Airline and situation.
Some Airlines do charge if a second seat is needed.
Some will not charge extra.
One Airline did have a charge per pound rate for passenger and luggage, It was not popular.

People shouldn't pay by mass if that's what you're asking. The airplane doesn't care. They will charge for extra cargo, though, because that takes up more limited space, also, not by mass, but by volume. If an overly obese person needs two seats, yes, they probably have to by two tickets.

If they are unable to fit in the seat, or the light attendant notices that their fat is hanging over the armrest into someone elses space, then they will have to purchase an additional seat
