If so, who was the last one?

    Yes, in chronological order:

    John Tyler (Whig, although he was expelled from the party soon after succeeding William H Harrison, who died 1 month into office)

    Millard Fillmore (Whig)

    Franklin Pierce (Democrat)

    Andrew Johnson (Democrat)

    Ulysses S. Grant (for third term) (Republican)

    Harry S. Truman (Democrat, briefly considered continuing for 3rd term, as the amendment passed during his tenure banning 3rd terms didn’t apply to him, but dropped out due to lck of support.)

    Lyndon B. Johnson (dropped out early in primaries due to lack of support).

    LBJ, 'decided' after his masterful 'performance' in 1964 not to run in 1968 considering the racial tension in Chicago with Black Democrat on White Democrat racial violence nation wide.

    Yes, but not recently. The last one who was eligible for another term but didn't get renominated was Lyndon Johnson in 1968.
