I know that Jadzia is actually an old guy...

Jadzia and Ezri were different people. However, they were both hosts to the symbiont Dax. Sisko refers to Dax as "old man", because Dax was inside a male host when they first met. Symbionts live much, much longer than the hosts, and must transfer to a new host when the current one dies, in order to keep on living. The symbiont retains i's memories when it moves to a new host.

Dax is a Trill which is a race of symbiotes. They join with humanoids. Jadzia and Ezri are humanoid Trills who host the symbiotes. Dax is immortal and has lived in several different hosts. That's why Sisko calls Dax, who is currently in the body of a young woman, "Old Man" because Sisko originally knew him in the body of an older man who was something of a mentor to him. The joined Trill symbiote possesses the accumulated memories but also has personality differences due to the new host.

The "old guy" is Jadzia's "symbiote" JAX, but for the life of me I don't recall Ezri, sorry
