The combination pill gives withdrawal bleeds on a very regular schedule. They aren't truly regular periods, though they seem identical so it's easy to see how people get them confused.
It is possible to have PCOS and still have regular cycles, although it's unusual. However as I said, since she's on the pill she doesn't really cycle at all.
Most people with PCOS can conceive, it just may take them a while longer or they may need some medical help. However someone taking the pill, PCOS or not, is probably not gonna get pregnant. That's the point of the pill.
