how are fake instagram comments made?

If an account gets repetitive fake comments, who, and how, are those generated? I know someone isn't manually making 50 accounts. So if you have a clue then feel free to go into details?

    Even if people aren't manually making accounts, if an account is hacked and taken over or simply made for the purpose, code can be run to simply spam text into comment boxes such as ads or shoutouts.
