The easy way to decide is to split the sentence into two parts.
He's spending time with his parents. He hasn't seen THEM in five years.
When you see 'them', use 'whom'.
Don't forget you need a comma after 'parents'. Without a comma, you're implying that he has two sets of parents, one set (A) that he hasn't seen in five years and one set (B) that he HAS seen in five years, and now he's spending time with set A.Using the comma avoids all this, making it clear that there is only one set of parents.

He is now spending time with his parents, WHOM he HADN'T seen in five years.
You can't say "hasn't" because he is seeing them now.
