Apart from not obeying the lockdown and social distance I think it is because Spain and Italy have a large population of the older age group who live within the younger family - like mum dad and grandma etc all live in same house as daughter and her family and some of the younger ones still had to go to work so a higher chance of the virus spreading?

Italy is a tourist spot (not just from China but also from anywhere).   When the govn issued a quarantine, most people didnt listen to the gov rules.   They stayed outside like nothing happened.    As a result, the outbreak took fast and in large number.  The hospitals also couldnt handle the new virus (unlike China who handled it pretty well).   So many died because the hospitals did not know how cure the patients.    In China, 75% were cured and out after a month.

Because they were slow to implement social distancing and self idolating.
