A... Howie would pay back 1.04d which is the principal, d, plus the interest .04. If your loan was for 1 dollar, then you'd owe 1.04 which is 1.04x1.
C... 200 x 1.04 = 208
Edit to add: The Anon poster says top question answer B simplifies to the same, but it does not. Again using my example of 1 dollar, 1.00 + 1.04 is 2.04. The answer is A.

Howie needs to pay back the loan plus 4% of what he owes tacked on as interest. 4% is .04 x whatever.
If Howie owed 1d he would also owe (.04 x 1d). Putting the two together gets you expression B. It simplifies to expression A.
Expression C gives you two copies of d, expression D is too large a multiplier.
For the second question, plug $200 into your winning expression.
