Yes. If you listen to the discussion between indian-looking guy and Jet, he refers to a belief that each person's life is guided by a star. And if you watch the credits to the very end you'll see a star fade out, symbolizing Spikes death.
Certainly a sad ending, but a very fitting one.

Hey, that star could have been for Vicious. It certainly looks like Spike dies, but he survived some pretty bad injuries earlier in the series, and I can't believe Faye wouldn't be there to pick him up.

According to the shows creator he did in deed die although i can not find the article in question now and regarding the movie it takes place in the middle of the TV series

It does look like he's dead, but then how is he alive for the movie? The way I see it is: either he survives or the movie is set before the final episode.

I don't remember if he dies after fighting Vicious, but if he survived, he was certainly messed up.
