Yes this is possible. The sperm are still good a varicocele just makes the sperm warmer then it should be. I dont see how artificial insemenation would work without getting the varicocele repaired first.

Even if the then Duchess of York had not liked sex it would have been easier for her to just "close her eyes and think of England" instead of subjecting herself and her husband to a medical procedure (and the inevitable gossip). The fact that it took so long for her to get pregnant suggests that there were physical and/or psychological problems on one side or on both sides which might have led them to seek professional help. A sensible thing to do.
