What information is listed on a death certificate?

The information varies, but usually:

name

date of birth or age

place of birth

parents' names

informant and their mailing address

occupation

marital status and spouse's name

cause of death

attending physician

place & date of burial

type of burial

It has the persons full name, age, date of birth, place of birth, race, father's name, mother's birth name, address, funeral home name & address, cause of death, doctors name, place it was filed, social security number, veteran or not, occupation, industry, sex. I worked in a funeral home for 8 years - typed many death certificates!

My grandfathers death certificate had date of birth, date of death, social security number, last known address

It should have everything you listed except the chidren's names, but the certificates are probably different from place to place. If you could, I would call the place you're planning to order it from and ask.
