I am trying to write a letter to the examination board for mitigating circumstances (mums ill in hospital).?

Dear Sir/Madam,

I am writing to inform you of mitigating circumstances which arose during my examination period. At this time, my mother was seriously ill in hospital (*add illness here if you feel the need for extra violins*); this caused a lot of upset in my daily life and may have affected my abilities in examinations. I hope you will take this matter into consideration during marking.

Make sure you include any documentation (e.g. letter from hospital or doctor) to back up your claim. You may wish to alter the above to suit your circumstances.

First find out who you need to address the letter to. You will need to ask your Examinations Officer for this information. They may already have spoken to someone at your exam board about what happened. In which case it would be better to ensure that you write to the same person. Your exams officer may also wish to send the letter for you. You need to ensure that the Exams Officer has a copy of your letter and your mothers discharge paperwork. She would have been given this at discharge. It will state her admission and discharge dates.
