Look at the chart that most treadmills have showing the ideal age/maximum heart rate for each 'zone' (aerobic, anaerobic, endurance). Increase the difficulty as you can, and maybe decrease the difficulty so you can work on your coordination.
For tightening up, you'll have to do some weight training. Does your gym have a 'circuit area' with a bunch of machines in a circle? That's a great and easy way for a beginner to weight training to work each body part in a safe way.
For more advanced weight exercises, ask your doctor about what you can/can't do. I'm sure they'll be thrilled that their patient is taking care of themselves and will gladly give you some pointers!

If you feel that 4.0 speed & 2 Incline is too easy then move the speed button to a higher number. It doesn't matter what incline you have the treadmill on, as long as you are comfortable. Good luck!
