Make a police report and get written statements from all the witnesses. Then turn it over to your insurance company. Work them to go after her. In fact I would insist they go after her for all the damages and costs including attorney fees and court costs. She has committed a crime, the sooner you get it on the books as such the better. If you give this woman an inch she will take a mile and you will end up buying her a new car. Seriously, dot all the eyes and cross all the tees. Others here are absolutely correct, by running she is hoping to get away with something, that attempt alone should tell you not to trust this woman. Smack her down hard before she has a chance to sue you for leaving your truck in her way.
I'm not sure how it works but it seems to me if your insurance company does their job and recovers ALL the costs, your insurance shouldn't go up. If it does talk to your agent, and the agent from another company or two as well.
EDIT: One more thing, fix EVERYTHING. I have let small stuff go and regretted it... Don't be a nice guy, she will turn you into a sucker.

you need to report it. dont wait another day. your insurance wont go up, especially if you were not at fault, however hers may. chances are if the crash only left a few scratches and no major damage - there probably isnt any real damage done to your truck as far as suspension and axles go - it takes a pretty hard hit to do that. good luck

Take the truck to the body shop of your choice and let them check it out. If it did not cause any damage to the suspension or axle then you are not worried about and you can just let it go. If the axle or suspension were damaged, you would probably feel it when you drove the vehicle.

There must be a police report filed if you expect damage to be covered by any insurance company.

I'd call the police and file a hit-and-run. If she hit your vehicle and didn't tell you or leave you a note, then that's exactly what it is. Treat it differently, and she'll try to worm her way out of it.

Report it. She had the opportunity to handle it outside of the office and skipped out. Point this out to her when she starts a hate campaign. SHE Ain't ON YOUR SIDE.
