when you forward the message put your name and the number on it in the message box before you send it i.e press forward. then type in the message box where you want to put ur name etc. then click the send button once you have put in the email addresses you want to send it to.

if you want it as a signiture rather than a one off (like a poll for example) then if you go into your email options there is an option for a signiture, write what you want and then ok it. that will be added to all emails that you send.

Hit the forward button on the email toolbar, select the people you want to send it to, write your name and number at the bottom of the list and then press send.

If the list is an attachment, you need to download the attachment, open it, add your name and number to the list and save it.

Then go back to the email, hit forward, then click add attachments. Attach the document you've just edited, chose the contacts you want to send it to and click send.
