It could be that you have fast metabolism. Were your parents skinny when they were your age? I used to eat and eat when I was that age and stayed the same weight all the time. Then I turned 25, got married and gained 45 pounds in 1 year bc my metabolism slowed down.
Also, when you say hungry do you mean like snacky hungry or voraciously hungry for a large meal all the time?
I would be concerned and seek a doctor if adding more calories does not do the trick. I would also consult a doctor if you are actually losing weight, despite eating all the time. It could be some sort of digestive disorder to that could be causing your body to not absorb all that it needs to.
Not trying to scare you or anything, just trying to help - bottom line - if it worries you, see a doctor.

You are underweight. Maybe you have worms or some medical condition whereby something inside you is eating up all your nutrients. You should see a doctor because it could be serious.
Especially if you start to loose weight.

try increasing your protein intake. for each sugar, eat a protein (glass of milk & piece of fruit or ham slices & yogurt or trail mix or cheese slices & apple sauce). eat smaller meals more frequently during the day. see your doctor to rule out a medical cause if this doesn't help.

hmm well if your diet is in check assuming you get enough protien and fiber to supress your insulin levels and appetite perhaps you have like a tapeworm or something similiar to that.
