Supervisor Performance Evaluation?

What type of questions do you think belong on an supervisor evaluation form.

some you may think it is wrong to give your supervisor a perfomance evaluation but this is something that I was asked to do by my supervisor. I am ONLY interested in the type of questions that employees would like to answer (professionally) on an evalution.

    Asked about Job Knowledge- If he is people person- Hi communication skills- Hi attitude towards workers- The way he handles problems- How positive is he towards company changes.
