You get out the larger ones with tweezers. The small and soft ones - from Opuntia f.e. - can be covered with some melted wax. Then cool with water and peel off the wax including the needles.

Here's a trick I read about sometime but I haven't tried. Get some white school glue or household glue and put a big blob around where the needles are. Let the glue dry. You know how it gets rubbery/plastic-like when it dries. Supposedly, when you peel the glue off your skin it will bring the needles out with it. It might be worth a try. If you don't have glue, you might try anything else that dries like that - cosmetic facial mask or something.
