Short answer is that it is damaged in some way meaning that it isn't being engorged due to damage to blood supply vessels. Short answer is to rest it, stop masturbating (especially rubbing the head) and it will repair itself but it will take time. And no there is no need for surgery.
Here is what I wrote to another guy:
It is usually caused by rubbing the head in masturbation instead of the shaft. Your penis responds by covering up the head so as not to damage it. Best way to cure phimosis is to cut down on your masturbation (for a while at least until the foreskin retracts). If you are to do it, don't rub the head.
If you pull the skin back and the eye is red on each side of the slit that is a sign of damage to the blood supply vessels If this is the case you have been masturbating too much (and with too much pressure).
As I say, the penis head is supposed to be as hard as possible which then helps it force it's up through the skin. If you damage it by applying too much pressure in rubbing the head, it becomes softer (also more sensitive) and it isn't hard enough to push through the skin. Often the erection isn't hard enough either - also due to damage to the blood supply vessels which occurs by applying overdue pressure often for prolonged periods of time.
Your penis needs time to repair. So go easy on the little guy. As your penis repairs itself you will notice how the head seems to be more engorged and consequently harder which will expand the foreskin. Your head will then eventually peep through.

I am intrigued by the term "cure". What exactly are you trying to "cure"? I seriously doubt you have phimosis. It is an extremely rare condition, and the term is generally used incorrectly to describe a tight foreskin. In which case, there is certainly nothing you need to "cure". Stretching and other torturous methods you describe, and others have mentioned also, are totally unneccessary. What you most likely have is a perfectly normal and perfectly formed penis. That fact that the foreskin is tight is not a problem and does not need any of the ridiculous suggestions (including your own) to remedy an imagined situation. The penis and foreskin work together in almost all men - each being unique and specifically designed for the individual owner. You cannot go around comparing yourself to anyone else and expect to find exactly the same penis and foreskin as yours anywhere. Nor should you compare yours to anybody else and expect that their is right and yours is somewho wrong. You have been misinformed about your foreskin and this has caused you some needless concern. I hope that you will now try to learn more about the wonderful thing your fully functional penis is, how to care for it and how to use it properly. Whatever you do, do not ever submit to the cruel and barbaric procedure of circumcision which is already outlawed in the most progressive countries of the world.

Phimosis is when the foreskin is too tight, preventing retraction. I would wager that the part that you refer to as still being connected is the frenulum, and it's meant to stay connected, it's located on the underside of your penis, and runs from the top of the shaft right beneath the head down. As for actually treating Phimosis, you can simply practice stretching it in the shower, as the water will help promote flexibility. You basically need to get an erection, then just slide the foreskin back as far as it can go without pain, and you should do this repeatedly. If you keep doing that every time you shower, you should gradually notice that you can pull it further and further back, until one day it will be full retracted. If that fails to help, just talk to your doctor about a cream he can prescribe, and please don't be afraid to ask your doctor, I guarantee he's heard worse, and your parents won't dig too deeply into it, as people these days hear something concerning another's genitals and they usually leave it be. Circumcision is not the only option, in fact, it should be the final option after all other channels have been exhausted, and those include the stretches, the creams, minor incisions to loosen the foreskin, and even partial circumcision, allowing you to keep some of your foreskin. So just try the stretches, then move on to the creams if the stretches don't work. You can probably ask your dad or even the doctor about this too, and they can more than likely give you pointers on how to remedy this without going under the knife. Good luck, and best wishes to you.

I have mild Phimosis, the main problem is all the Smegma that buils up under the foreskin. My treatment is to rinse under the skin with a 10ml syringe filled with tepid water; holding the end like a balloon to agitate then wash all the gunk out. Works a treat!

This is common among young teens and guys who have not tried pulling their foreskin back much, that's because the foreskin is attached to the head when you're young to prevent the head from getting dirty. Usually it separates before puberty, but in your case you have to give it a little help. Now that you probably masturbate and pull it back to wash under it (only takes about less than 10 seconds) it'll start sliding back more and more. If you have a tight foreskin, you can actually stretch it like I did when you shower. It only took a few weeks and it hasn't given me a single problem ever since. =)
Work on getting it less tight. Here's what you do. When you shower and and you're soaping up your body, pull the foreskin back the most you can without forcing it, and then wash the head with the other hand. If you do this each time you shower for a few weeks, you'll see results. It'll slide back a little more every few days.
Now, as for the sensitivity, as studies have proven, the foreskin keeps the head sensitive. That's not a bad thing, since that creates pleasure. However, you don't want it too sensitive. Yours is just too sensitive because it hasn't been used much before. So you just have to rub it gently at first for some weeks (start off with just water and then every few days try to rub a little more) and you'll see the difference. Then you'll be able to rub the head however you want. =) So in a way, those two things are tied.
So yeah, work on it and in a few weeks you'll like it. =) And of course, feel free to e-mail me from my profile if you have any questions!
You do NOT need circumcision. And you have every right to remain uncircumcised, as after all, circumcision does carry risks and most 'benefits' are unfounded or contradicted. Common risks of circumcision are more difficulty with masturbation (it was made popular in the USA to stop masturbation, after all), and lower sensitivity. There's a reason why the USA is the last developed nation doing it to newborns, and our rates are falling; it's as low as 14% in some states by now. Research it more in the link below:

In the end, if the stretching fails after a few weeks, you can ask a doctor for a cream. It's 0.05% betamethasone, which is successful in over 90% of cases. You can even take the study to him. It's a really inexpensive cream; Wal-Mart sells it for $4 with prescription and online you can buy it for about $10 at drugstore.com, for example.

But focus on stretching it first. That should give you results after some time.

If you can pull back the foreskin to expose the head then you're fine. If it's uncomfortable or doesn't go all the way back, even if it's only when you're hard, then that's probably mild phimosis. An operation usually isn't necessary, because stretching solves the problem. Just keep pulling the foreskin back daily as far as it will go and it should loosen up. If that doens't work you can see your doctor, they can prescribe a cream that will loosen the foreskin.

If that someone was not a doctor, I would go and see one. A medical professional is the one to ask.
