I HAVE to put my 7 yo son in the basket part of the cart. If I need to shop for an extended period of time thats where he goes. Because he is autistic and wants to run away. He thinks its funny. Im not leaving my son at home because it would be "easier" for me. A lot of things could be "easier" for me but I deal with it and move on. A few years ago what people like you thought bothered me and made me stress when making a trip to the store.. Because you dont know the story of why people have kids that behave the way they do, Sure, some of it may be bad parenting, but with autism growing every year ( 1 in 166 kids will get autism) you may be seeing stranger and stranger parent/child interactions as time goes by. He's not going to wear a sign and i'm not explaining it to every jerk in the store that gives me their looks of death. My son looks just like any other kid. But you'll figure it out....
