In some states you do, but the list "friedokra99" posted is from 2004. It is not current. For example, Oklahoma no longer requires a blood test for a marriage license. Best advice is to call your local court clerk who will be issuing your marriage license. They will know all the requirements (blood tests, id, waiting period, etc).

Not in the state of MN or WI they dont. We even asked but where told if we had been together for 2 yrs or more and had been tested in that time and no worries it wasnt needed. Also if you had a child in the time together you get tested then again not needed.
Some other states might still require one, talk to your local County office they can best tell you whats what.

Inquire specifically regarding your state.
While most states do not require them anymore, it is a good idea for reasons other than transmittable diseases; for example, to check for blood type compatibility, which can effect pregnancy.

A few states still require it. They are: District of Columbia (ok, not really a state, I know) Georgia, Indiana, Massachusetts, Mississippi, Montana, and Oklahoma.

Not in my state, but it may be required where you live. Call the court house and find out.
