Correct way to measure floor registers?

Just bought a house, the floor registers (vents) are old and I want to replace them. I measured the largest side of the vent, but the vents I see for sale do not match the measurements I got. What is the correct way to measure these?

    Don't eyeball it, measure the inside of the vent. Or do what the last person suggested, take one with you and compare with others. I'd take a tape measure and make sure you get the exact same size vents.

    It s on the line of a 2x4.. A 2 1/4 x 12 1/4 opening requires a 2 x 12 register. Like the 1 st guy sugg, take an old 1 w/ you. With an older house the size may be harder to find but they had the sizes out for a long time so look around. GL

    I just took one of the old ones with me to the store and got replacement ones that were the same size. From what I recall, the ones we have had the dimensions of the overall piece, not necessarily the hole size.
