    It really counts on your case load, and the general population of the nursing home. Contrary to popular belief, not ALL people in nursing homes are the elderly. Sometimes it is individuals who are younger who need long term care or are in late stages of certain diseases.

    Facility social work is going to be similar across the board, at least loosely, with differences existing from agency to agency. For the most part, as a social worker, your responsibility will be to attempt to link the individual to services either within the site/network or outside of the network, and coordinating comprehensive care. This might involve making sure speciality clinic visits are coordinated, things are in line with Medicare/Medicaid (unless your agency has an eligibility coordinator or something like that), or coordinating other particular needs (like psychiatric care).

    While this is very general, and might not be helpful, its going to differ based on your organization.
