The three main issue which are significantly affected by early ambulation in patients who have been administered a subarachnoid block are:

Postdural Puncture Headache – incidence related to use of larger needles (22G), cutting needles. The leaking of the CSF due to trauma to the dura can cause a significant postdural puncture headache. Having the patient stay in a flat position decreases the CSF pressure on the dura allowing it to seal. Occurrence can also be reduced by rotating the needle so that the bevel is pointed to the side, this decreases trauma to the dura.

Transient Radicular Syndrome/Transient Neurological Syndrome – self resolving pain aggravated by early ambulation post-op.

Hypotension. Many patients have issues with hypotension following subarachnoid blocks, the prolonged period lying prone allows the body to resolve this issue prior to ambulation, thus decreasing the risk of injury due to falls.

    Hope this helps.
