How do I change my daughter's last name?

DO I need her father's permission? She currently has his last name and I want her to have it changed to my last name. He does not really have an active role in her life, so I don't see the point. We were never married.

Update:

I'm not letting "my hate seep into my child's heart". She is the one that wants it done. Don't make ASSumptions!

    you need his permission he is still the child's father and has right not if he sighed his rights Away but if he didn't you need his permission and you need to go to the courts and ask there for a form to have a name changed

    Regardless, he's still her father. Wait until she's old enough to make the decision on her own. Don't let your hate seep into your child's heart.

    I think you only need his permission if you have Joint custody.

    But, if you have full, it's up to you.
