I think that it just means someone from the country. My grandma told all of us our whole life that we were hillbillies- and the only difference that I saw between myself and kids at my school is that we lived out in the country. I dressed the same as them, had the same interest, except for maybe spending more time outside.
The dictionary states-a person from a backwoods or other remote area, esp. from the mountains of the southern U.S.

Hillbillys were originally from Appalachia, the term is usually used to describe country folk.

Jeff Foxworthy defines a hillbilly as a person who is "gloriously unsophisticated" watch the guy on tv he is freaking hilarious.
